HOSPITAL WHOLESALE ORDER FORM

Here's How It Works:

• Minimum Quantity of Screen Print Pieces via Wholesale is 40.
• Once you have submitted your order, additional items will require a new wholesale order.
• We will contact you with final price of each item by the next business day.
• We will begin your order once we have  total order cost and your approved artwork.
• We will most often deliver your order within 14 business days from date of deposit and artwork approval and will notify you of any holdups should they occur.
• We do not do "Rush" orders.  We will help you out in any way that we can, but we don't guarantee orders in less time than above since we strive to be 100% on time with our orders.
